The Deadline to Submit a Certification Form is 11:59 P.M. EDT on October 17, 2025.
Eligibility:
You may be eligible for a payment from the Pinnacle Fair Fund if:
- You paid management advisory fees during the Relevant Period;
 - Your Recognized Loss calculates to a distribution amount equal to or greater than the Minimum Distribution Amount of $20.00, as calculated under the Plan;
 - You are not an Excluded Party; and
 - You timely submitted a valid Certification Form.
 
You are excluded from participation in the Pinnacle Fair Fund if you are an Excluded Party as defined in the Plan.
Considerations and Preparation:
To be considered for eligibility for a Distribution Payment from the Pinnacle Fair Fund, you must provide the information requested in this online Certification Form and submit a release through the Fund Administrator’s online claim system by October 17, 2025. Failure to do so may result in your claim being rejected and you being precluded from any recovery from the Pinnacle Fair Fund.
Timely submission of your Certification Form does not guarantee you will be eligible for a Distribution Payment from the Pinnacle Fair Fund or that you will be compensated for your claimed loss.
Please fill out the Certification Form completely. After submission of the Certification Form, you may be requested to provide documentation to support your Certification Form. Failure to completely fill out the Certification Form or timely provide all requested documentation will result in rejection of your Certification Form.
Supporting Documentation:
If you are filing a Certification Form on behalf of the Claimant, you must provide proof of authority to act on behalf of the Claimant.
If you are not a U.S. Person, you must also submit a completed IRS Form W-8BEN, W-8BEN-E, or other W-8 series form, which can be found by visiting the following IRS website: www.irs.gov/forms-instructions.
